A doctor advised me decades ago to keep a record of my meds (amt, dosage, time) and past medical history. To this I added all my past meds that I was prescribed for narcolepsy but didn't work. The meds we get for our narcolepsy often have wicked side effects and we don't want to buy the same real estate twice. I keep a complete medical history on me at all times and I don't fill out those sheets either. I just give them my sheet. I am so forgetful, but I also have a slew of stuff to remember beyond sleep issues and meds. This is a great recommendation for others. I highly recommend it as well. Everyone should keep as much info as they can about their medical history on them at all times.
